Requests for name changes for individuals must include the name on file with LINN, your social security number and copies of documentation supporting the change (e.g., marriage certificate, divorce decree, etc.). Requests can be sent via mail, fax or email to the appropriate contact listed below. |

Requests for name changes for companies or corporations must be accompanied by a certificate of merger, name change or other applicable documentation. They must also include the company's new tax identification number (if different). Requests can be sent via mail, fax or email to the appropriate contact listed below. |
